One of the most common problems in endodontic practice is to arrive at the correct working length. The use of three-dimensional diagnostic tools such as CBCT, microscopes, ultrasonic and all the technical advancements available today improve the outcome of the most complex situations that dental professionals face in the everyday practice. Starting from sound biological principles and a comprehensive analysis of up to date endodontic scientific knowledge, the wise use of the latest technologies and most advanced clinical procedures will be explained using an explicative case series. Technical details and clinical tips and tricks to simplify and make the treatment more predictable will be described. Both complex primary treatments and retreatments will be addressed from the establishment and maintenance of glide path and apical patency, even in complex anatomies and calcified or blocked canals, to the apical preparation and refinement. Different solutions will be illustrated with the final aim to guarantee the predictable and durable maintenance of natural teeth. Endodontic microsurgery procedures will be discussed, describing the techniques step by step together with original solutions and tips and tricks developed by the author.
